Memory lapse in SuperMemo indicates a situation in which the user receives a failing grade. SuperMemo uses grades to indicate the degree of recall. Failing grades indicate that the answer to a particular question has been forgotten. Questions with many memory lapses are called leeches. They are hard to remember, and usually they are poorly formulated.
